2012 Registration

Step 1: Fill out and print forms

Download, fill out, print, and sign the following forms. Please note you will need the Adobe Acrobat Reader to view/print these files. You can obtain a free copy of the Adobe Acrobat Reader from the Adobe website.

To download these files to your computer, right-click on the form and choose “save as…” from the menu that pops up.

All forms are interactive PDFs, which means you can complete the form fields on your computer but for legal reasons you must print, sign, and mail the forms to us.

Registration and Release From Liability Form (required)
Authorization for Medications (optional)
Authorization to Release Child to Care of Others (optional)

Step 2: Submit Payment

A minimum $25 registration fee is required at the time of registration. If you would like qualify for the pre-pay discount rate include your full payment at that time of registration. See our pricing page for more information. Otherwise we will bill you for the remainder of your camp fee after we receive your registration.

Step 3: Mail

Mail registration forms, full payment, and $25 registration fee to:

After we receive your registration, we will send you back a confirmation letter with additional camp information.
